METHODS OF PAYMENT
Registration fees are in United States Dollars (USD).  Payment in other currencies will not be accepted.  The registration fee can be paid by credit card, bank draft or bank transfer.  Registration must be accompanied by all registration forms.  Registration is only valid upon receipt of payment.

All payments must be made using the following options:

Bank Draft (including bank charges) payable to Philippine Veterinary Medical Association.

Bank Transfer (including bank charges) transfer of registration fee to the indicated bank.  Please fax and/or mail or email proof of payment to the 16th FAVA Congress 2011 Secretariat.

CONFIRMATION
Once the registration form and payment were received, an acknowledgement letter and receipt of payment will be issued by the 16th FAVA Congress 2011 Secretariat.

CANCELLATIONS AND REFUNDS
The 16th FAVA Congress 2011 Secretariat must be notified in writing of cancellation of registration.

  • By September 1, 2010: Full refund of registration fee less 10% for administration and bank charges.
  • By December 31, 2010: 50% refund of registration fee.
  • After December 31, 2010: No refund on registration fee

If you are unable to attend the conference, a substitute delegate can be accommodated at no extra charge provided a written notification is received before September 1, 2010.
